Please provide as much information as possible. You may remain anonymous however please keep in mind that more information helps us to better identify issues and make changes where needed.

If you are sharing about a coworker who is not following safety guidelines, please share their name and details of your concern.

Please keep in mind that an incident report should be completed for more serious concerns such as breaking quarantine placed on a staff person by the Department of Health, COVID 19 exposure or serious concerns for personal health and safety. If in doubt ask your supervisor or an ELT member.
